A sneak peek into one of your most valuable tools

Metopio’s Community Health Atlas was designed for public health professionals that spend too much time hunting for data and not enough time using it. In short, it’s a living, breathing ecosystem of reliable, ready-to-use data designed to help you make faster, smarter, more confident decisions.

The Community Health Atlas combines the latest federal, state, and local datasets into one intuitive platform, putting the full picture of community health — from chronic disease to social determinants to whatever else you might be looking for — at your fingertips.

In just a few clicks, you can explore population trends, compare geographies, and uncover the underlying factors that shape community well-being.

With the Community Health Atlas, you can:

  • Integrate data across hundreds of sources to track trends over time

  • Standardize metrics across teams using a consistent, reliable framework

  • Benchmark performance between regions and identify gaps or disparities

Whether you’re preparing for your next CHA, drafting a CHIP, supporting a grant proposal, or just curious about the state of public health in your area, a Community Health Atlas makes data accessible and actionable.

From Insight to Impact

Good data doesn’t just inform; it inspires. The Community Health Atlas helps translate numbers into stories and evidence into strategy.  With built-in tools, you can:

  • Create customizable visualizations to highlight local trends

  • Access raw data for deeper analysis and research

  • Share maps and charts that engage partners, boards, and community stakeholders

Each visualization is designed to be clear and compelling — ideal for presentations, reports, or community engagement sessions!

Built-In Collaboration

Community health work is inherently collaborative, and the Atlas makes it easier than ever to bring partners into the process.

Through private workspaces, teams can analyze shared data, align strategies, and design programs rooted in evidence. It’s a platform that grows with you and your team — flexible enough for local projects, powerful enough for statewide collaboration.

  • Invite cross-sector partners into secure workspaces

  • Share dashboards and reports with health departments or hospitals

  • Align program goals with real-time, geographically relevant data

The engine behind Metopio’s Community Health Atlases powers “Health Compass Milwaukee,” a dynamic, publicly accessible data platform designed to centralize and contextualize community health information. Health Compass Milwaukee was created by the Milwaukee Health Care Partnership (MHCP), a public-private consortium that brings together the region’s four major health systems, plus public health departments, federally qualified health centers, and academic and community-based partners to align efforts around a shared mission and strategic community investment.

By partnering with Metopio, MHCP is able to provide an accessible, community-informed data tool so all partners can access disaggregated, real-time insights down to the census tract level.

A Real-World Example: The Chicago Health Atlas

When the Chicago Department of Public Health partnered with Metopio to modernize the Chicago Health Atlas, their goal was simple: make neighborhood-level data easier for everyone to access and use.

The result was transformative. Today, the Chicago Health Atlas serves all of Chicago’s community areas, offering interactive maps, filters, and integrated social determinant data that empower policymakers, organizations, and residents alike.

It’s more than just a hub for data, it’s a public resource driving advocacy, funding, and measurable progress across one of the nation’s largest cities.

The Future of Community Intelligence

From Chicago to counties and coalitions across the country, Community Health Atlases are helping public health teams see what matters and act faster on what they find.

With Metopio, you can go beyond reports and spreadsheets to create a living framework for data-driven collaboration, accountability, and impact.

Because better insights don’t just build better reports, they build healthier communities.
