Yale School of Public Health’s PopHIVE and Metopio Partner to Turn Real-Time Community Health Data Into Action
The Yale School of Public Health today announced a new partnership between PopHIVE (the Population Health Information and Visualization Exchange) and Metopio, a national community health data automation platform. Together, they aim to make modern health data easier to use where it matters most: in decisions that affect patients, families, and local communities.
PopHIVE is a Yale-led public-good platform that provides timely, trustworthy, de-identified population health data. It brings together information from clinical care, public health systems, and other community sources. The goal is simple: help people understand what’s happening in their communities and respond faster.
Metopio works directly with local health departments, hospitals, and community organizations across the country. Its platform helps leaders move from static reports and spreadsheets to clear, decision-ready insights. It’s purpose-built for collaboration, efficiency, and impact, empowering organizations to navigate community health challenges and drive meaningful change.
Through this partnership, PopHIVE’s dashboards will be integrated into the Metopio platform, enabling local health and public health leaders to combine high-quality national data with their own community context to make informed decisions that improve health outcomes.
Importantly, this is a two-way partnership: in addition to helping deliver PopHIVE data “where decisions happen,” Metopio will work with PopHIVE as a thought partner on community-centered design, including how to incorporate and use social determinants of health data in practical, actionable ways.
What the Partnership Enables
PopHIVE and Metopio will work together to:
Bring trusted data into everyday decision-making. PopHIVE’s de-identified, multi-source health indicators will be embedded directly into tools that local public health and healthcare teams already use.
Turn data into action. Pairing national trends with localized insights will help community leaders better identify risks, close gaps, and respond faster than ever.
Strengthen the use of community context data. Metopio will help ensure that measures such as housing, income, education, and access to care are presented in ways that are practical and actionable.
Design with communities in mind. Both teams will focus on building tools that are clear, usable, and centered on improving real people’s lives.
Initial areas of focus will include respiratory diseases, childhood immunizations, chronic diseases, and injury and overdose, with plans to expand over time.
